Application for Admission

From this page you may download the College's application for Admission in .PDF format or choose to submit an online application (subject to it's date restrictions). If you don't already have the free Adobe Acrobat Reader program please click on the link below to download and install it. You'll need need the Reader to be able to view and print the hardcopy application. The instructions for submitting a hardcopy application are listed below. The instructions for the online application are included with the form.

Online Application for Admissions - SUMMER SESSIONS

  1. Online Application for Admission - SUMMER 2008 (3 sessions being offered: June 2-13; June 16-July 3; July 7-August 21)
  2. Available for SUMMER 2008 new/transfer students from 3/14/08-6/30/08. 
  3. New/transfer student online applicants must pay a non-refundable $20 application fee by VISA/MC over the web.
  4. New/transfer student online applicants must also have a valid e-mail address.
  5. All new/transfer Summer Session students must choose non-degree status only (LZ98) from the curriculum drop down menu.

Online Application for Admission - Fall 2008

  1. Online Application for Admission - FALL 2008
  2. Available for FALL 2008 new/transfer students from 3/14/08-8/28/08.  Classes start Tuesday, September 2, 2008.
  3. New/transfer student online applicants must pay a non-refundable $20 application fee by VISA/MC over the web.
  4. New/transfer student online applicants must also have a valid e-mail address. 

Additional Instructions

Please supply the following items to the Admissions Office:

  1. Completed and signed Application for Admission
  2. $20.00 non-refundable application fee
  3. Final high school transcript, copy of high school diploma, or GED
    If you are currently enrolled in high school, have your Guidance Office send the
    most up-to-date transcript followed by a complete official transcript after graduation.
  4. Proof of Measles/Rubella Immunity (if born after December 31, 1956) Connecticut State Law requires that all full-time (degree seeking and non-degree/ non-matriculating) and part-time matriculating students enrolled in post-secondary schools be protected against measles and rubella. Students must have two doses of measles vaccine to ensure adequate immunization. The first must have been administered after January 1, 1969, and the second after December 31, 1979 (at least one month apart). For rubella (German Measles), one does of vaccine is considered adequate immunization for all students. Students who graduated from a Connecticut public or private high school from 1999 and after are exempt (must provide proof of completion).
  5. If you have previously attended college, please submit official transcripts for
    evaluation and award of transfer credit where applicable, as well as meeting
    prerequisite requirements.
  6. Financial Aid/Veteran’s Tuition Waiver
    If you would like to be considered for financial aid, you must complete the FAFSA
    (federal form). You may obtain the FAFSA online at www.fafsa.ed.gov.
    Also, if you will be receiving a veteran’s tuition waiver, you must notify the director
    of financial aid.
  7. Students with Disabilities
    If you have a physical or learning disability and would like to learn more about
    services offered at QVCC, please contact the director of learning services at 860-412-7200.
  8. Mail or bring your completed application with the $20.00 fee, transcripts, and
