For proper registration please fill out both Emergency Medical form the with the Authorized Pick-Up List, and the Behavior Agreement. Both of these forms can be printed from the links provided, filled out and either brought in to the CPL Office or mailed to us. These are very important forms for registration so please make sure your child has them.
